Your patient comes in presenting with a slightly strange looking cornea. They seem to be in no pain, and yet the cornea at a distance looks like it is sloughing down.
Whilst observing their cornea from above, it does not seem to be pushing the eyelids out of the way.
last chance
Under a slitlamp, you note that there is no stress lines nor are there any brown rings.

You also note that there are no crescent ulcers, however, the cornea thins dramatically near the bottom peripheral region, leading to the sloughing shape of the cornea above the thinning.
the
answer
Pellucid Marginal Degeneration
- Similar to keratoconus, but instead of sparing periphery, spares the central cornea
- Will not show Vogt striae or Fleischer rings, but may show Mooren's ulcer, which is a crescent shaped ulcer.
- Whilst looking at the cornea at a tangent, the peripheral thinning will occur at either the 4 o'clock of 8 o'clock region
